First Appointment

Before your first visit, please print out the intake forms and practice policies to the right, fill out one set for each person coming to the appointment, and bring them with you.

Please allow at least 24 hours notice for any cancellations.

Sessions are tailored to individual needs and agreed upon before the appointment. Payment is required at the time of service. While I don't accept insurance, I can offer a receipt for self-filing purposes.
